The First Signs of Capsular Contracture After Breast Implants
Oct 1, 202627 min
Months or years after surgery, one side can suddenly feel firmer than the other, and most women assume they did something wrong.Dr. James Koehler and Dr. Vincent Gardner walk through what a mild case actually feels like, how it's graded on the Baker scale, and why some cases never get noticed until something changes years later. The leading theory is a "slime layer" of bacteria that settles on the implant and triggers chronic inflammation, which is part of why implants are more often placed under the muscle today.Treatment carries its own risk of recurrence, so Dr. Koehler explains how an acellular dermal matrix like Strattice dramatically improves the odds of it not coming back — plus what rupture, rippling and bottoming out look like, and why the "replace your implants every 10 years" rule isn't quite true.Read more about capsular contracture treatment: easternshoreplasticsurgery.com/breast/breast-implant-revision/capsular-contracture-revision Questions answered by this episode: What does capsular contracture feel like when it's just starting? Facelift: What Recovery Really Looks Like, Day by Day
Sep 17, 202617 min
Most people know what a facelift looks like on the outside. Almost nobody talks about the weeks in between — the swelling, the numbness, the moment a worried spouse finally exhales. Dr. James Koehler walks through the real timeline: the 24-hour dressing change, why he skips drains, when stitches come out, and how long bruising actually takes to settle. The most counterintuitive part might be the six-week mark. Many people look camera-ready by then, but the last of the swelling under the chin doesn't fully resolve for three to six months — and comparing a one-week photo to a friend's isn't a fair test of how surgery went. Koehler also covers what almost never comes up before surgery: temporary numbness around the ears, the rare nerve bruising that can affect a smile for a few weeks, and why spouses tend to worry more than the person having the procedure. Body Lift: What Recovery Really Looks Like, Week by Week
Sep 3, 202619 min
Six months. That's about how long it takes for swelling to fully settle after one of cosmetic surgery's biggest transformations — and the first ten days feel entirely different from month six. Dr. Vincent Gardner walks through the day-by-day reality of a full body lift: soreness, three to four surgical drains, and a new non-narcotic pain protocol that's changing how people get through the first week. He and Dr. James Koehler cover the parts nobody warns you about — where swelling pools in unexpected places, when it's actually safe to drive again, and why most people end up grateful for the scar once the excess skin is gone. Dr. Gardner also breaks down the real timeline for seeing your final shape — and why the first glimpse in the mirror is only the beginning. There's no such thing as erasing a mark on your skin. The best a cosmetic surgeon can do is trade it for a thinner, flatter line placed where a waistband or bathing suit hides it — which is exactly what a tummy tuck does with an old vertical C-section scar. Dr. James Koehler and Kirstin Jarvis get into the honest gap between removing a scar and improving one, and why every operation leaves a mark no matter how skilled the hands that made it. For a cosmetic surgeon, where that line sits and how it heals is the whole job. They clear up one of the most common mix-ups in scar talk: a keloid is not the same as a thick, raised scar, even though most people — and plenty of physicians — use the words interchangeably. The two heal for different reasons and get treated in completely different ways, from steroid and 5-FU injections to, in stubborn cases, low-dose radiation. Dr. Koehler explains why location and skin tension decide whether a scar fades to almost nothing or stays visible, when it's worth re-operating versus treating from the outside with lasers and microneedling, and why time is the most underrated healer of all. He tells two stories — including a woman convinced her beautiful breast reduction scars were ruined — that show how much of the scar conversation is really about expectations. Questions answered by this episode: Can you completely remove a scar, or just move it somewhere hidden? What's the difference between a keloid and a hypertrophic scar? Why do some scars fade beautifully while others stay raised and thick? How do surgeons treat keloid scars? Do drugstore scar creams work as well as medical-grade silicone gel? When is it worth having a scar surgically revised? How long does it take for a surgery scar to fully fade? Why is a tummy tuck scar sometimes thicker than a scar on the face? Do microneedling and laser treatments help flatten and lighten scars? What should I know about scarring before booking a cosmetic surgery consult? You did the work and the pounds are gone, but the skin didn’t disappear. Dr. Vincent Gardner walks through what a full body lift actually does after major weight loss: how a lower body lift combines a tummy tuck with a buttock lift and an auto-augmentation so you don’t end up flat, and how an upper body lift erases the back rolls with an open book tummy tuck that carries the incision all the way around. He also clears up the timing questions people get wrong. You no longer have to wait a full year at a stable weight, it doesn’t matter whether you lost the weight with GLP-1s, bariatric surgery, or diet and exercise, and the single biggest predictor of how well you heal isn’t the surgery at all — it’s whether you’re getting 90 to 100 grams of protein a day. For anyone living inside leftover skin, Dr. Gardner makes the case that this is less about the scars and more about walking out feeling like a different person. Questions answered by this episode: What is a 360 body lift and how is it different from a tummy tuck? What is the difference between a lower body lift and an upper body lift? What is an open book tummy tuck? Do I need a buttock lift and auto-augmentation after weight loss? Should I have my tummy and buttocks done at the same time or separately? How stable does my weight need to be before body lift surgery? Does it matter if I lost weight with GLP-1s, bariatric surgery, or diet and exercise? How much protein do I need to heal after body contouring surgery? How long is recovery after a 360 body lift? Will body lift scars fade, and how do I make them look better?
